Welcome to the Health Information Management Department. It is our job to make it easy for patients and care givers to have access to their medical records through our Patient Portal. If you are a patient and currently do not have access to your online portal all you need to do is call the office and provide an email address to receive a personal invitation. Your online patient portal gives you on-the-go access to:
- Look up past encounters
- Send instant messages to your provider
- See test results
- Share records with other providers
If you are unable to access the Patient Portal, and you are looking for your medical records, please download the Medical Records Release authorization form below, complete and return it to us.
Completed forms can be mailed or faxed to the Medical Records Department at the address listed below. Due to the global pandemic, we are not currently accepting walk-ins to pick up records.

Agency / Counsel Requests
If you are a government agency or legal counsel requesting records or enquiring on the status of a request, please send an email to firstname.lastname@example.org or call us at 609-278-5900. If you have made a payment but have not received the records, please include a copy of the check for efficiency. Thank you.
