We take great pride in offering medical, dental, behavioral health, and pharmacy services to all patients, and have discount programs available to help offset the cost of care. To participate in our services, the first step is to become a patient. Becoming a patient starts at registration which can be completed when you schedule an appointment or complete a new patient registration. If you prefer not to complete this form online, you can also download this form as a pdf and email the completed form to registration@henryjaustin.org.

Established patients are reassessed at the time of an appointment to update any changes in patient status.
