Chronic Care Management (CCM)
To help our patients manage their health and wellbeing, we are now offering Chronic Care Management (CCM) services. Any patient who has Medicare and has more than one chronic condition is eligible.
If you have chronic conditions, CCM is an important piece of the care that we can offer you. If you participate in CCM, you will meet regularly with a dedicated clinical team member and have access to other providers to help reach your health goals. If you need further assistance, we will coordinate with other health specialists and community partners.
Some benefits of participating in CCM include having a dedicated team member to oversee care and goals, keeping on track with appointments and medications and much more.
Eligible chronic conditions include, but are not limited to:
- Arthritis
- Asthma
- Cancer
- Chronic obstructive
- Pulmonary disease
- Depression
- Diabetes
- Heart disease
- High blood pressure
- Infectious diseases
- (HIV/AIDs)
- Pain

Location of CCM: Available at all Henry J. Austin Health Center sites and also available through telemedicine.
On-site Lab: 321 North Warren Street, Monday-Friday, 6 a.m.-2 p.m.
On-site Pharmacy: 321 North Warren Street, Monday-Friday, 8 a.m.-6 p.m. | Saturdays, 9 a.m.-1 p.m.
