Senior Health and Disease Prevention Program
We work to educate the senior population of Mercer County, particularly the medically underserved, regarding healthy life style choices and disease prevention.
As a program of Henry J. Austin Health Center, this program is able to direct individuals who are in need of services to one of our health centers and the many departments that they can benefit from.
Senior Health Services offered* include a program in the following areas:
- Chronic Disease Self-Management, Stanford University developed program
- Diabetes Self-Management, Stanford University developed program
*all Title111D, the highest level of evidence-based program.
