School-Based Services Through Our Mobile Health Unit

Thanks to grant funding, the Mobile Health Unit provides school-based health services in partnership with the Trenton Public School District, promoting health equity among children and adolescents who unjustly experience disparities in outcomes because of race, ethnicity, or family income. The MHU provides children with immunizations, preventative screenings, annual physicals, and obesity prevention services while fostering an atmosphere of love and trust. By serving patients where they live, work, and play, the Community Mobile Health Initiative (CHMI)  has created a hub for wellness that is a trusted source of preventive healthcare services.

Partnership with Vision To Learn

In partnership with Vision To Learn, a complete program designed tohelp students from Pre-K through 12th grade gain corrective eyecare services that will help them succeed in school and life, HJAHC isproviding prescription glasses to every child in Trenton who requires them, free of charge.

Vision To Learn mobile clinics solve the problem by bringing eye careand glasses to children at schools in lower-income and under-resourcedcommunities. This partnership will screen 12,000 students whose vision needs would otherwise go untreated, providing prescription glasses to every child who requires them, free of charge.

Clinical Pharmacy Services

In 2016, HJAHC established the first clinical pharmacy in Mercer County, providing medication management, counseling, and education for service area patients. HJAHC’s innovative leadership through integrated healthcare–an intensive intervention and follow-up approach is shown to significantly improve health outcomes and quality of holistic care, which contributes to patient satisfaction, improved access to services, and increased quality of effective care.

Behavioral Health

Behavioral health services at HJAHC are integrated into primary care, increasing overall access to behavioral health care; improving health outcomes for patients with mental illness and/or substance use disorders; improving health behaviors such as compliance with treatment and reducing overall health care costs.

Dental Services

Our locations serve Medically Underserved Areas, with a critical shortage of primary care physicians, dentists, or mental health providers. HJAHC provides integrated dental care and is the only dental provider in Mercer County to accept payment on a sliding scale based on income.
