Adverse Event Near Miss
An event or situation that could have resulted in an accident, injury, or illness but did not, either by chance or through timely intervention.
Adverse Patient Event
An undesired outcome or occurrence, not expected within the normal course of care or treatment, disease process, condition of the patient, or delivery of services.
Building Maintenance Issues
Facilities hazardous conditions (wet floors, broken elevators, broken furniture).
Code Report
Code report (patient resuscitation, combative, hazardous materials, smoke/flames, etc.)
Crash cart - other problems (not opened for meds)
Crash cart is unlocked other than codes & maintenance.
Disruptive Patients or Family Members
Disruptive behavior (using profanity, arguing,).
Employee/Workplace Injury or Illness
Use to capture additional data regarding a workplace injury or illness.
Environmental and Facility Safety
Reporting of ideas for prevention or actual incidents or accidents (unlawful or not) pertaining to environmental or safety that might result from normal or abnormal operating conditions.
Fraud
Allegations of deliberate deception to secure unfair or unlawful gain. The purpose of fraud may be monetary gain or other benefits by employee or patient.
Harassment (including sexual)
Allegations of conduct in the workplace or other professional setting that creates a hostile environment. Sexual harassment includes unwanted sexual advances, request for sexual favors or obscene remarks that causes a feeling of intimidation.
Inappropriate Discipline or Retaliation
Allegations of inappropriate disciplinary action or retaliation related, including for reporting incidents.
Major Event/Incident
Emergency evacuations - power outages - diasters.
Medication Administration Error/Medication Order Error
Wrong medication administered/Wrong medication ordered.
Other Incidents
Any allegation or concern that does not fall under any of the other categories.
Patient Falls or Injuries total (adults & children)
Patient falls or injuries.
Policy Violation
Allegations of an employee who violates organizational policy, procedures or or the Code of Conduct.
Potential Breach of Confidential or Protected Health Information
Potential impermissible use /disclosure of confidential information, including PHI. Either shared inappropriately or a result of unauthorized access to or collection of the information, that compromises the privacy of affected person / patient.
Staff Needle Stick Injury/Exposure to Body Fluids
Staff accidentally get stuck with a needle.
Theft / Burglary
The unlawful taking of company or personal property (embezzlement, burglary, etc) by employee or patient.
Workplace Violence
Act(s) of aggression, physical assault, threatening behavior or verbal threats that occur in a professional setting or context and causes physical or emotional harm to customers, coworkers, or managers.
