MAJOR FUNCTION:
The Community Health Worker will ensure patients link with and/or refer clients to other services, programs, and agencies as needed and promotes importance of social supports and good lifestyle choices. These services will be provided for an extended period of time across the continuum of care to achieve an optimal clinical outcome. The CHW is also needed to drive HJAHC’s latest site the Mobile Health Unit, which is a clinic on wheels.
 
ESSENTIAL FUNCTIONS:
• Drive and maintain the Mobile Health unit in sanitary conditions; which includes daily operational checks, daily logging and cleaning after every outreach and clinical event.
• Provide exceptional service and/or care to all patients, customers and team members. 
• Registration of patients at ICS/MHU sites as needed
• With appropriate HIPPA training and patient consent, communicates with family members and significant others as indicated.
• Promoting effective resource utilization with the multidisciplinary team. 
• Assist with scheduling transportation for identified patients scheduled for follow- up evaluation interviews. 
• Identify beneficial local resources and keeping track of patients referrals and assisting with keeping appointments.
• Promotes the program within the Health Center, at other health care organizations that feed and draw from the Health Center.
• Assists in the collection of Program data.
• Attends team meetings with the program personnel and clinical staff to report and review program progress.
• Contacts key personnel at participating centers and other service/health-related organizations with access to target population to identify status of services and coordinate patient care.
• Manages all aspects of the patient navigation activities within the Health Center, other centers and in the community.
• Ensures requisitions for non-clinical office supplies, equipment and forms are made in a timely manner and available at all sites, work with the on-site team to identify these items and ensure requests are made and approved for purchase. 
• Regularly updates the program coordinating staff.
• Provides reports according to instructions from patient navigation program coordinator.
• Perform other duties as requested by Unit Manager/ Supervisor.
 
EDUCATION
• High School diploma.  
• Training as a Community Health Worker; Medical Assistant acceptable, Certified Medical Assistant Preferred; minimum one (1) year related experience and/or training in a healthcare setting or relevant combination of education and experience.
• Must hold current certification in Basic Life Support (BLS) Certificate.
 
PREREQUISITES FOR THE JOB
• Experience with driving a Van is preferred
• Bilingual (English/Spanish) required.
• Ability to read and comprehend instructions, correspondence and memos. Ability to write correspondence; effectively present information in one -on –one and small groups to clients and other employees of the organization.
• Computer literate.
• Skill in the application of modern office techniques and practices and the use and care of office machines and equipment. Working knowledge of common software packages necessary to perform the functions of the position.
• Ability to review and abstract data from patient records.
• A valid New Jersey driver’s license. CDL is preferred but not required
• Access to vehicle is required to work at the different sites, attend workshops and meetings.
• Some evening and weekend availability is required. 
• Committed to a smoke-free environment both personally and professionally due to the vulnerable respiratory health of the population served, and to reflect the commitment to respiratory health education provided by the program.
