ADVANCED PRACTICE NURSE
Reports to: Chief Medical Officer                                                  Division: Primary Care Services

MAJOR FUNCTION:
Under the general supervision of the Chief Medical Officer, provides clinical services defined within the New Jersey Board of Nursing guidelines for Nurse Practitioners. Perform medical functions in accordance with legal and judicial mandates; performs technical applications to meet state and federal standards and regulations; does related work as required.
ESSENTIAL FUNCTIONS:

  • Manages a panel of patients and works to ensure Same Day Access and continuity for the panel of established patients managed.
  • Is a member of the clinical team and works with the clinical team which may include a nurse, medical assistant and receptionist to manage the patient panel.
  • Actively participates in mandatory daily team huddles, this includes arriving to huddles on time.
  • Performs comprehensive or episodic medical assessment of patients with acute and/or chronic mental health problem, in accordance with protocols established by the Chief Medical Officer and/or designated collaborating physician.
  • Orders and evaluates diagnostic studies; develops patient treatment plan and goals; prescribes treatment and medications keeping within protocols established by the Health Center and within the guidelines established by the New Jersey Board of Nursing.
  • Advises patient of need for lifestyle changes, tests, and medications, maintains an awareness of interactions of medications and advises patient of the results of non-compliance with treatment plan.
  • When appropriate, determines the need for physician, sub-specialty, and/or emergency room consultation/evaluation.
  • Maintains complete and adequate medical charts, follow HJAHC procedures for documentation to ensure appropriate billing.
  • Provides professional direction and acts as a resource to other staff.
  • Participates in quality and clinical peer reviews and serves on quality and clinical committees.
  • Monitor and supports the activities of any students assigned to the respective department.

 

  • Follows HJAHC procedures for documenting patient records to ensure appropriate billing for client treatment.
  • Greets and interacts with all patients/clients/customers via telephone or directly in a personal and professional manner; promotes positive patient and community relations; acts as member of the health team.

ADDITIONAL RESPONSIBILITIES:

  • Participates in other assigned duties as designated by the Chief Medical Officer.

EDUCATION:

  • Graduation from an accredited college with a Master’s of Science degree; is certified as a Nurse Practitioner by the American Academy of Nurse Practitioners or American Nurses Credentialing Center; is licensed by the State of New Jersey as a Nurse Practitioner

PREREQUISITES FOR THE JOB:

  • Must hold current and valid certification from an accredited certifying body as noted above
  • Must hold current and valid NJ Nurse Practitioner licensure, CDS and DEA licensure
  • Capable of working with computers and software applications suitable for health care environment; excellent interpersonal and communication skills
  • Good health and freedom from disabling physical and mental disabilities which would impair the proper performance of the required duties or which might endanger the health and safety of oneself or others
  • Ability to read, write, speak, understand or communicate in English sufficiently to perform the duties of this position. American Sign Language or Braille may also be considered as acceptable forms of communication.
